Submit Cross-Connection Survey
Cross-Connection Surveys are due by September 31, 2026 so that the District may complete its service-area-wide hazard assessment within the timeframe required by the State. Every customer must complete the survey applicable to their property and return it to the District. To learn more about why you received this survey and what cross-connection control means for you, click "Read the District's Letter on Cross-Connection Control" in the box to the right.

Download the survey that matches your property type — residential or commercial — fill it out completely, and then choose how you'd like to return it.
